The cost is $250 an hour
(minimum of one hour and thereafter in 15-minute increments)

You have 2 methods of paying:

• Shopping cart (which now includes a PayPal option you may prefer)
• By fax, send us your credit card detail

After paying the fee you will be told what to do. It's important you get me plenty of data, so that you will get the maximum value out of the phone session.

TERMS AND CONDITIONS OF SERVICE

In signing up for this phone consultation service I hereby acknowledge and understand that Dr Keith Scott-Mumby, not withstanding his many international medical degrees, is not a licensed medical practitioner in the State of California. It has been explained that he may not legally diagnose or treat any medical disease or condition and I may confer with him only in the capacity of a nutritionist and health educator.

In all medical matters and those relating to personal health issues I agree to consult with my own licensed health practitioner and/or doctor, who may not be usurped.

Furthermore, I agree to hold harmless and indemnify Dr Keith Scott-Mumby and any family members, agents and employees of his from any and all liability, actions, causes of action, injury, disease, death, damages, negligence, debts, claims or demands of any kind and nature whatsoever (including attorneys’ fees and costs) by myself or any person, which may arise now or in the future, by or in connection with my decision to confer with Dr Scott-Mumby.

Furthermore I agree to hold Dr Keith Scott-Mumby harmless and indemnify him and any family members, agents and employees of his, concerning any misunderstanding, mistake, incompetence, inadequacy, falsehood, misinterpretation or confusion of any kind with any consequence, no matter how prejudicial, arising from any educational advice and instruction he may give me, whether verbal or in writing, now or in the future.

In signing up for this service I hereby intend to be legally bound by the terms of this document.  It is further agreed that the release expresses a full and complete settlement of liability, regardless of the adequacy of the aforesaid health education and that the acceptance of this release shall not operate as an admission of the liability on the part of anyone, nor as estoppel, waiver, or bar with respect to any claim that Dr Keith Scott-Mumby may have against you, the client.

This is a voluntary release for any and all future damages, injuries, accidents or death. The risks include those foreseen and unforeseen, known and unknown.

This release is binding on my heirs, executors, assigns and administrators.

I also accept the policy of no refunds given.
